To advance a new model for community health to thrive based on relationships, joy in work, and health equity.

Our goal is for our patients to feel listened to, have enough time to ask necessary questions, and be able to discuss what’s most important to them with their provider. In order to accomplish this, we offer hour-long initial visits either online or in-person and 30-minute follow-up visits. Additionally, we understand the need for accessible primary and urgent care when a health concern arises, which is why we offer same-day visits (by appointment, for our established patients) for children and adults, and expedited new patient visits. Our model of integrated care means we have an onsite Behavioral Health Clinician and a Community Health Worker as part of our patient's care team.

Student pricing

Our partnership with the Oregon Trail School District provides no-cost health care to k-12 student children and adolescents in the district. These include students at Cedar Ridge Middle School, Oregon Trail Academy, Naas Elementary School, Welches Middle School, Firwood Elementary School, Kelso Elementary School, and Sandy High School.
